48-Hour Aging Parent Decision & Planning Guide

PERSONALIZED FAMILY PLANNING SERVICE

A clearer family roadmap when an aging parent’s situation changes.

Get a personalized aging parent care plan that organizes the decisions, questions, family responsibilities, and next conversations you need—without sorting through everything alone.

RN/LNHA-informed and professionally reviewed. Educational decision preparation—not medical care, assessment, or case management.

When an aging parent needs more help, this guide can help you…

  • preparing for a hospital or rehabilitation transition;
  • considering more help with everyday life;
  • comparing help at home or residential options;
  • coordinating family roles from near and far; or
  • unsure what to ask or whom to contact next.

This service organizes decisions and conversations. It does not assess your parent or decide what care is needed.
